Patient Questionnaire

When uploading a study you will be prompted to add the patient's questionnaire information. This should appear as a pop-up window during the physical uploading of data from your ARES device. Here are some questions you might have regarding this process-

 

How do I add the questionnaire to a study after it has been uploaded?
 
  • Go to Home Sleep Testing> Pending Studies Queue

  • Click Search to generate a list of studies in a pending status
  • Locate your patient's name and click the Green Plus Sign on the patient name
  • Click Add Questionnaire, enter the information and click Save

Why would I not be able to add a questionnaire to a study?

The questionnaire can only be added if the study has NOT been interpreted by your assigned reading physician AND it is an available status. After uploading a study it will remain in a process pending>processing status for approximately 30 minutes, and during this time you will not be able to add a questionnaire.

What happens if I cancel the questionnaire pop-up?

Unfortunately, you will have to wait for the study to appear in the Pending Studies Queue. Once available you can follow the instructions for adding a questionnaire.

The pop-up window did not appear. What can I do?

If the pop-up window did not appear, first double check all Internet Explorer windows to confirm the pop-up did not open behind another window. Then click HERE confirm your Pop-Up Blocker Settings are configured correctly. Unfortunately, you will have to wait for the study to appear in the Pending Studies Queue. Once available you can follow the instructions for adding a questionnaire.
